The more we know, the healthier you can be.

We are now improving our programs to be sure you have the information that is best for you. A person's genes and culture can make him or her more likely than other people to have a disease like asthma or diabetes. To do this, we need to know more about you.

Soon we will begin asking for your race, ethnicity, and the written and spoken language you prefer. This information will not be shared with anyone, except your doctors. They also need to know you better.

If you help us learn more about you, we can help you get the care you need to find diseases early. This will help you take control or cure these diseases.

You do not need to share this information. But please consider helping us give you better health care. You do not have to answer these questions. But, if you do, your answers will help us make sure that you and your family gets good care.

Your information will stay private. We will only share it with your doctor and other healthcare providers. For more information about how your race, ethnicity and language information is protected and stays private, please read the privacy protections for member race, ethnicity and language data.
